Becoming a Client

NCAP serves any HIV-positive person living within the NCAP designated service area - Larimer, Logan, Morgan, Phillips, Sedgwick, Washington, Weld, and Yuma counties. Our services are free and confidential. Some services may have financial eligibility requirements defined by grant funders.

At your initial appointment, you will need to bring a copy of your HIV/AIDS diagnosis from your healthcare provider, or have it sent to us via fax. In addition, many of our services have eligibility requirements, so we will need your proof of residence and proof of income if any and proof of health insurance. If you are having difficulty in obtaining these documents, a case manager can assist you.
